Cool Math homepage
Cool Math is targeted to math learners from 13 to 100.  This homepage has links to related sites including:
     Cool Math for Kids designed for ages 3 - 12
     Cool Math Games for Little Kids for ages 3 - 5
     Cool Math Games for those over the age of 5

"Where you learn math by playing with it." 
Targeted to High School and Undergrad students (algebra and up), although the basic algebra section may work for middle school. 

Lessons for grades 5 and up.  This site uses Google Earth and "you will find lesson ideas, examples, and downloads for mathematics that embrace active learning, constructivism, and project-based learning while remaining true to the standards."
  
Requires registration and is free for the first 10 days.  After the 10-day trial it is $39.95 per year.  The scope of the site includes Math, Science, Social Studies, Language Arts, Art & Music, Life Skills, World Languages,  Research and Career Skills, and Health and Phys. Ed. 

Lots of interesting stuff – Escher, Fractals, lots of great links
